The Ice Islands consist of Mejborn, Neve, and Winterbay, along with two small unnamed islands.
You can go to any of the three main islands taking the boat, south-west of Thoris (here 
).
The price is 20 gp from the continent to any island, 7 gp island-to-island, and the passage back to Medivia is free.
